Help Me Solve A Mystery

I know I have been asking for your help a lot lately but this is one puzzle I can't figure out on my own. And I know when Ellis comes this weekend he is going ask where this person heard about his request.  All I know is a last name...Chambers and a city...Provo.  Anyone have any ideas?
